    We will always do our best to provide support in the shortest possible time of receiving your request. Outside of our office hours, Monday - Friday 8.30am to 5:00pm. After-Care provides an emergency after hours service to coordinate support for life's unexpected emergencies. This service is coordinated by a highly experienced case manager with a BA in disability studies, social welfare and masters in health services (aged care).

  • Does After-Care help people living in the community with dementia?

    Approximately 200,000 Australians are diagnosed with dementia. After-Care has specialised services to help people continue living in their familiar home environment with dignity and care. Our team delivers personalised services designed to help people maintain their daily lives at home, sometimes visiting 3 times per day or staying overnight if necessary.

  • I have a child with a disability. Does After-Care help children and younger people?

    Yes, we have many years of experience providing personal care, social support and in-home respite for children and younger adults with a disability. After-Care employs carers with disability training and also provides carers for the Royal Children's Hospital RCH @ Home program. After-Care provides support services to people receiving Individualised Support Package (ISP) funding from the department of human services and also provides care coordination and case management when required.
