Form 15 is a written description out lining the type of glass either being supplied or onsite/existing, verifying thickness, lamination and if heat treated or not by using laser gauges, completed by the installer or by a person with suitable knowledge.
Yes they can be renewed. Depending on the type of rubber seal the entire glass with it's metal frame may need removing for a new seal to be fitted. There is a risk the glass pane, if under size in thickness may crack. Though all care is taken, this will become an added cost if the glass cracks.

The springs that help the window sash to open and shut as in double hung windows have probably snapped. The spring sits inside a plastic tube either side of the sash. These springs are called spiral balancers. When the spring inside snaps the sash is often difficult to lift open and will often slam shut making it extremely dangerous for unsuspecting fingers. Modern houses have spiral balancers.
Yes. Apart from being painted shut there is a good chance the rope cord (sash cord) that hold the counter balance weights (skinny cast iron weights) have both snapped and the weights are still inside the wall cavity. Replacing the cord is quiet involved, the internal window architraves will need removing, and the external architraves removed if the top sash needs sash cord replacement.
Once I have come out to take exact measurements of the window frame. It will then take 7 to 10 working days for the manufacturer to cut and toughen the required glass panel. All glass being fitted into bathrooms must be toughened or laminated glass. Laminated glass doesn't come obscure, generally it is only clear.
I will require rough measurements of the broken glass panel. I will then bring bigger glass panels than required. Once at your property I will remove the broken glass, remeasure the exact size required then cut a new sheet of glass to suit.
The maximum height I will work at off an extension ladder is 4meters. Any higher will require the client to supply scaffold or a high lift boom.
